The Behavioral Health Services Center in conjunction with our Diabetes Department, goal within MHSI, is to redesign our current system of care based on the Care Model's six components. We have an evidence-based, culturally competent system of depression and diabetes management for African-American and Hispanic residents of North Milwaukee. We will increse our recognitition of depression and diabetes and improve treatment outcomes through the following mechanisms:
Depression:
70% of patients identified with CSD (clinically significant depression) will be offered 1st line medication treatment for depression, at least 6 months of psychotherapy or the combination of both.
50% of patients identifited with CSD will have at least clinically signignifacnt improvement of PHQ score (5 pt or greater improvement in PHQ Score) or demonstrate response scores on the PHQ (50% improvement of PHQ score) within 6 months.
70% of patients identified with CSD will have documented self-management goals in the medical record.
70% of patients identified with CSD will be reassessed between 4-12 weeks from the baseline PHQ score.
70% of patients identified with CSD will be assessed for past history of trauma.
